OARA Mini Minds Registration Form

I hereby give consent for my child to participate in the Coding and Robotics Camp organized by OARA Youth Innovation Lab. I understand that the camp includes activities in science, technology, engineering, arts, and mathematics (STEAM), involving the use of LEGO® Education SPIKE™ Prime, Scratch, and Python tools.

I consent to the installation and use of educational software required for participation in these activities.

I understand that I may list any special needs, medical conditions, or allergies separately in the designated form fields.

Photography and Media Consent:
In accordance with the Freedom of Information and Protection of Privacy Act, I acknowledge that OARA Youth Innovation Lab may capture photos or videos of participants during the camp. I understand these may be used for educational or promotional purposes. I will indicate my media consent choice in the provided selection.

Acknowledgment of Risk:
I acknowledge that while all necessary safety precautions will be taken, participation may involve inherent risks. I release OARA Youth Innovation Lab, its staff, and volunteers from liability for any injury, loss, or damage that may occur.

Emergency Medical Authorization:
In the event of an emergency, I authorize OARA Youth Innovation Lab to take appropriate action to obtain medical care for my child, including administering first aid or arranging transport to a medical facility.
